After a stroke, the hospital and inpatient rehab get you safe enough to go home. But home is where the real work begins: the stairs, the shower, the walk to the mailbox, the kitchen you used to cook in without thinking.
At Empower Rehabilitation & Wellness, we bring stroke rehabilitation to you. Every visit is one-on-one, in your home, with a Doctor of Physical Therapy who specializes in stroke recovery.

Jaime Pierson, PT, DPT, is a Certified Stroke Rehabilitation Specialist (CSRS) through NeuroRecovery Unlimited. Stroke rehab isn't one of many things we do on the side. It's why this practice exists.
Why stroke rehab works better at home:
No transportation barriers. Many stroke survivors can't drive yet, and getting to a clinic two or three times a week wears out the whole family.
We train the tasks you actually do. Practicing a transfer onto your own toilet beats practicing on a clinic mat.
Family can be part of every session. Caregivers learn how to help safely, without doing too much or too little.
We see the safety risks clinics miss. Loose rugs, a missing grab bar, a tricky threshold. As a Certified Aging-in-Place Specialist, Jaime can recommend practical home changes.
What we work on:
Walking and gait training, including with a cane, walker, or AFO
Balance and fall prevention
Strength and coordination on the affected side
Arm and hand use for everyday tasks
Transfers: bed, chair, toilet, car
Stairs and getting in and out of the house
Endurance and managing post-stroke fatigue
Dizziness or vision-related balance problems
A home exercise program you'll actually do
Is it too late to improve?
Probably not. Many people are told progress stops at six months. Research shows stroke survivors can keep making meaningful gains in walking and function with the right, targeted practice, even years later. If you or your loved one still has goals, let's talk.

What to expect:
Your first visit: We talk about your goals and watch how you move in your own home: getting up, walking, stairs, and the bathroom.
About 60 minutes, one-on-one, every visit, with the same therapist every time.
Your doctor stays in the loop. They receive an evaluation summary within 48 hours.
A plan built around you, with regular progress updates and a direct line to your therapist.

FAQs
Does Medicare cover in-home stroke therapy?

Yes. We are in-network with Medicare Part B. For other insurance, we provide a superbill you can submit for out-of-network reimbursement.
Do I need a referral?

Not to start. New Jersey is a direct access state, so you can see us for up to 30 days without a referral. After that, we coordinate with your physician.
How is this different from home health?

Home health is for people who are homebound. You don't have to be homebound to see us, and our visits focus entirely on your rehab goals.
